Clinical Liaison

What do they do?

  • Act as the bridge between hospitals, patients, and post-acute care providers (such as rehab, home health, or long-term care).

  • Assess patients for appropriate placement, coordinate discharges, and educate both patients and families about care options.

  • Represent their organization to referral sources.

If you’ve ever been the nurse making sure your patient’s discharge actually made sense, you’ve already done part of this job!

Why Nurses Are a Strong Fit:

  • Strong clinical assessment skills to determine patient eligibility and care needs.

  • Compassionate communication with patients and families navigating complex decisions.

  • Experience collaborating with interdisciplinary teams and advocating for safe transitions of care.

Typical Qualifications:
  • Active RN or LPN/LVN/RPN license.

  • Acute care experience (especially med-surg, ICU, or case management).

  • Strong knowledge of discharge planning and community resources.

  • Excellent interpersonal and marketing skills.

This role is ideal for nurses who love patient interaction and care coordination but want to step away from bedside work. It blends clinical expertise with relationship-building and offers a meaningful way to influence care continuity.

Remember, nurses can do jobs that don’t have the word “nurse” in the title!
